Which form do you need?
If you were not at fault in the accident If you wish to make a claim for expenses in excess of $5000.00 or you think you will incur losses for more than 6 months, complete the Personal Injury Claim Form available from www.maa.nsw.gov.au the Motor Accidents Authority web site. This form needs to be lodge within 6 months of the accident.
